When choice is most important, you have access to more than one million in-network health professionals anywhere in the United States.

• The doctors who practice at Kaiser Permanente Washington locations
• An extensive network of contracted community providers in Washington
• First Choice Health network (in Washington, Oregon, Idaho, Montana, and Alaska)
• First Health network (in all other states)

Summit PPO also includes Kaiser Permanente Washington pharmacies, as well as the OptumRx network of pharmacies throughout the nation. Note: You’ll have lower copays for office visits and most prescriptions if you use Kaiser Permanente Washington providers and pharmacies.
